I need five cheap designers today (0 review freelancers accepted) - repost

This is a trial project in order to allow me to select five (05) freelancers. I've already found one but still need four more. This test project will be paid (but not much money). It is supposed to be done in two hours, for $2 - $3 /hr , total $6 ( max ). I know your hourly rates, but I need first to select five people to do this test for now; after that I will make bigger projects with higher rates. Here is the form : (1) [login to view URL] I do not like it. I need something BEAUTIFUL, more MODERN and PROFESSIONAL. I like something similar in form, but far better in design, than this : (2) [login to view URL] (scroll down the page toward the bottom). There's no need for a picture as the background, but need to include a "wheel" containing text at the top right of that form. So you need to download the form (1) , and keep all the texts and javascript codes (validation), but CHANGE THE DESIGN. Only the form, no need for the menu and sidebar / header / footer ... Just the form with the texts inside it. If you can make the form (1) beautiful, then send me a message, so that I can award five freelancers to create five different forms. What I need from each worker is one form : html, javascript, css, and pictures if any. I do not need php, as the webpage supports only html and javascript / ajax. If you must use PHP, please make it a separate file and make the url absolute, so that I can put it on a different server. I think that's clear. If not, try to google. This project needs to be completed today (before midnight, GMT), so if you can meet that deadline, please make a bid of not more than $3/hour and 2 hours of work.
