A form i created on my site must send data to my Facebook page, and send an email...HELP

I do have designs... However, without worrying about design, I'm in need of expertise. My code needs to be reviewed, debugged, and enhanced. My code is html, css, javascript, and php. I'm looking to land on something solid, very soon. So I need someone who isn't afraid at looking at someone else's code, dive-in, great at problem solving, and easy to work with. I'm not worried about the design at this time. However, when it comes to design (ui/ux), I encourage creativity and will compensate for you executing on your creativity. Therefore, it's important that you are confident. Instead of talking about it, I rather compensate you for what your ui/ux skills is worth.
