Redesign website - CSS3/AJAX/PHP work

I am looking for a web-designer with good aesthetic sense and designing skills, expert in CSS3, Javascript (including ajax) and PHP, to re-design my site http://pollwidget.org.
Some of the main tasks are:
Add an feature to choose/specify poll category (topic).
Add feature to add a thumbnail image to each poll answer option.
Show latest polls and most voted polls on a section of the front page with some elegant layout (Currently they are on a separate page [url removed, login to view] ).
Give an admin feature to hide/delete polls.
A search feature to search for polls by keywords.
For commenting on the poll we will use Disqus - developer needs to create/change the layout of the page to accommodate the comments section.
Display more data on the poll results page, like - poll creator's profile etc.
Use more prominent and elegant buttons and graphics on the site.
Add a points system (details later).
Some minor miscellaneous changes that will be communicated to the qualified bidders.
I am looking for excellent designers (css/ajax, html5) who have enough knowledge of PHP to integrate with the backend using robust and modular code. It will be an added advantage if you are a facebook API expert. Kindly specify your experience in this.
