Data Gathering on Web Application security vulnerabilities

I need some detail information about various attacks on web applications in recent years(not network attacks) and how the attacks were conducted, what remedy would have been in place and some snapshots of the attacks. the data should be between 40 to 80 records. the various column needed are detailed as follows: 1. Name of Website compromised 2. Type of attack (SQL injection, Xss, etc) 3. Date 4. Detail description of how the attack is conducted( including tool if any) 5. Snapshots 6. Reason why the attack was successful 7. What remedy is needed to be put in place in the website to prevent the attack 8. defences
