Simple form install and randomizer on wordpress site

2 things on site [login to view URL] 1. The form on the front page is not operational and I would like it replaced with something that looks like the image attached. No slider anymore. Just a form for state and a for for zip. The state selection should take you to the States-SEO page (this is a custom post type). The zip code should work as is for now it is bringing results within the 50 mile radius of that zip from this page - [login to view URL] . 2. Where every management company and vendor has a logo.. I would like a small bar (say 90px high) at the bottom above the bottom menu to randomly display logos from management companies and vendors. I think about 5-6 across would work. It should randomize with a different set of logos after each page refresh.
