Remove 2 lines of code on 2 seperate pages and fix banner on blog pages

I have a website that was injected with suspicious malware and need to have the injected code removed from my website. Below is the name of the malware and I will have to have it removed off of the two pages that google webmaster tools is seeing it on but I would also like to check the other pages to be careful. Also 5 blog pages have the banners not aligning correctly if that could be fixed as well that would be greatly appreciated.
