Word Doc and PDF- AED 1001 - repost

I have a scanned copy of a paper form that I need converted to a Word document and a PDF. Additionally, need the client logo and address added to head of document. PDF Document must have form fields to allow filling and saving. This is a repost of the project, the first freelancer did not complete the work satisfactorily in that the final deliverable became an image with form files overlaid in PDF. I need a quality document, like for example [login to view URL] The final PDF form must occupy only 1/2 half of an 8.5x11" sheet, so that 2 per sheet can be printed. This size requirement is due to how the client uses the form, placing it in a tray that only accommodates the half sheet. All form fields must be consistently sized, and easy for the user to fill. The Word document must have the form 2 times, at top and bottom, so that the client can print and cut the sheet in half, thus producing 2 forms. Attached is the final deliverable ([login to view URL]) from the last freelancer, this is for reference purposes as the size is good and all text is good. The parts that are not good- 1) Test is not crisp because this is an image, not a word document. 2) Fields are inconsistently sized, and not aligned well. 3) Instructions fields is not multi-line, that will make it hard for user.
