Build a pop-up contact formular in our shopify shop. PSD

We are building an online gaming store and are looking for a skilled and communicative developer who can help us with our vision. You should be experienced with shopify developing (CSS, liquid, HTML, JS) and have customized themes before. Please have good english skills. Develop and add a responsive fully functional “pop-up contact formular” to our shopify shop. It needs to be customizable (dropdown content, see briefing in attachment) and look exactly like shown in our .psd file. You need to make a version for mobile and desktop.
