Design a Website Mockup

I want an overview design\mockup of a small electronic medical records web application. The basic pages would capture the following: Table Purpose Demographics Demographic values (name, birth date, gender, race, ethnicity, address, et cetera). Problems A list of problems and diagnoses Allergies A list of allergies associated with the patient. Medications A list of the medications that the patient is or has been on. Immunizations A list of the immunizations that the patient has been given. Procedures A list of procedures that have been performed Encounters A list of encounters that have occurred Results A list of lab test results the have been generated VitalSigns Vital signs that have been recorded. FamilyHistory Family history associated with the patient. SocialHistory Social History observations associated with the patient. Orders Non-medication orders that have been made. Rx Medication orders that have been made. Additionally, Transactions and billing histoy would need to be captured. Most importantly though, it should have a dashboard where all people that is medical staff, family etc can upload information on the progress off care. Pictures, video text (kind of like facebook or a blog). The dashboard would have a timeline but would be linked to that patients electronic medical record ID. In terms of look and feel. I would like it to look like windows tiles to navigate around while displaying important info in the tiles themselves. I do not need database design or anything like that I just need conceptual look and feel and page flow etc.
