dvd template wanted for illustrator (see file)

hi, i'm looking for an illustrator template for a dvd packaging called DELUXE SOUND AND VISION. See the image. I need this template to be in layers as well as in CM/MM format (not inches). i will not pay for this as i only need something that you already you have. i do not want someone to make this for me. in return i will give a perfect rating and also pay for the GAF fees. See video capture also. The slipcase and casing is important as it opens out and I can see that it is folded inwards also. The template provided has to be the printers production type so that they can cut to size and fold. The booklet is not important if you don't have it as it is easy to do.
