It have to be working like this.

1. On google form: Show Upload Image button

2. On google response form: Show Image on response sheet or Image link somewhere (link to google document or other location)

Hi can help. You can't insert upload image button in usual way into google forms via from editor, but you can create google form as a web app and then insert image button. Regards, Viktor
