I urgently need a "quote box" for my wordpress site.

I need a qoute box to go in the header of my wordpress site.
Can be very simple at first.
* must be able to pull base quote information from a specific website, provided format is
{"ITEM1: 0.0001, "ITEM2": 1000.0001} It's the 1000.0001 from item 2 of the object that i need to quote on.
Visitor must be able to provide a quantity and percentage (ie 4%) and have the quote change to reflect this.
View picture for details. I must be able to edit to add disclaimers etc.
Bonus if it includes an "Email quote" button, that emails the quote to the person and cc's my email address, and includes the users phone number for me as well.
I need this as soon as possible.
