Ethereum Dapp / Smart Contract Generator - Cryptocurrency & Blockchain

I am looking to have an Ethereum Dapp made that will allow users to do the following via a web url:
- deposit Ether into a smart contract
- input a text memo (optional)
- input expiration date/time in the future
- input a hash value (public key)
- input an additional ETH address
If the user returns prior to expiration and enters the private key funds will be returned to the originating address.
Otherwise funds will be disbursed to the alternate ETH address they supplied upon DApp creation.
Please provide references to any other smart contacts you have created and provide a final quote and timeframe for a working prototype.
