Hi Sir/Madam, I have been working at Office 365 for 7 years. I have experience Sharepoint, Project, PowerApps, Flow, Teams, PowerBI, and Planner. I developed many mobile/tablet app like inspection app, daily worksheet app, trade monitoring and handover protocol app with Flow. The pre requisites would be a license to build canvas apps, a SharePoint license of online or on-premise.... We can build reports in PowerBI but I would never suggest PowerBI for data storage. If you have the above mentioned licenses, your powerapp would automatically work on mobile. You do not need additional development or work in Android or IOS. the project should not take long to develop. Do you have a detailed business plan in place for this form or do you have to build that too? I would suggest having thought of the complete process from end to end and preparing some level of requirements in a document so that your development is faster and less expensive.

Hi, I would like to bid for this project and looking at the scope this can be delivered in max 5 days. I assume that other than capturing survey details thru manual and automatic inputs as mentioned in details, there is no other requirement on making any dashboard or report. I recommend to push survey details in SP list. Thanks, Raj
