Wordpress Plugin eForm Matrix Elements / Export data whit user meta Key values filters

I need an experienced (OR NOT) person with the Eform wordpress plugin and Exporter for eForm, plugins used for this job. Basically I need to implement questions with matrix elements in response like this, [login to view URL] (this work have 72 questions in 15 groups), i need this groups progressing by slides or pages like this [login to view URL] (next, prev). Each question and answer has a value and in the end a formula will be made or in the reports will need this values. Another important point is that before starting with the questions, we need the user select 3 user meta key values defined by us: Company, Department and Position. What we need is to get reports with filters by company, department and position. Show the overall average per department response, for example. I see that the plugin allows you to answer anonymously, will it be possible to implement this anonymity for these questions, but that necessarily select the 3 custom values: Company, Department and Position. I don't want request a username or email, if possible, just register the ip for unique ID and the NEW 3 USER META KEYS for the filters data export. I give you access to Wordpress or FTP if necessary.
