Wordpress: Generate "PDF Contract" from a Form

We are using WPLMS for Wordpress. The elearning site will allow students to take short courses and quiz after which they will receive a certification. We want to add an automatic data entry form to the "Course Completion Page" (or linked to the completion page) The user will enter the data for the form and submit. Upon submision, the data from the form will be displayed on a flattened pdf contract and sent via email to admin. We have the contract templates alread created with spaces for the data fields. The pdf will be displayed on the screen of the user for printing or saving (right click is fine). The Admin should be able to recreate the pdf contract (via admin panel) at any time with the data collected from the form submision. The user will enter the data for the form and submit. We will need this done for the 1st form and contract BUT will need to be able to easily create new forms associated with other contracts/documents. There will be new forms and associated contracts developed constantly. There are existing plugins (ie Gravity Forms) that can do this for $30/year but we are looking for a unique solution.
