Survey Form Manager

I need to develop a complete website for having feedback about services provided by car dealers. 1) Mistery Shoppers will visit the dealer, once they finish the visit, they will login on the website and fill surveys with several questions and text boxes where they provide feedback. Survey will have a progress bar based on the number of questions, and the results will be stored on a mysql database for later use on charts reports provided to Car Dealer Managers. Mistery Shoppers will have on their profiles a registry with all the surveys filled and timestamp. They will also receive a confirmation email every time they finish a survey. 2) Car Dealer Managers can login on the website to check results associated with their shops. They will obtain all questions the Mistery Shopper answered. They will also obtain different scores based on categories and several charts (line, column, pie, etc.) comparing their results against top 5 Car Dealers shops. 3) There needs to be an Administrator dashboard with the following features: - Ability to create survey campaigns: Register the questions, types of questions (multiple predefined values, numeric score, text, etc.) and how many steps will be included in each of the survey campaigns. - Admin must be able to add/delete/update users accounts (username, password, type of user, survey campaigns associated, etc.).
