2 small HTML5 features

Hi I have two small tasks for an experienced web-developer. That should not take longer than a few minutes. 1) I need a website template (quiz): two folders Pictures1, Pictures2 where I can put screenshots of websites or emails. Then a nice looking question field something like "is this website nice" and an answer option; yes, no, don't know. Then when someone clicks on yes - its green and says correct and goes to the next one. if clicked on the wrong choice then highlight red and automatically goes to the next one. At the end they should be a summary show the reasons why the answer was wrong - for example: textfield: you made the wrong choice because xyz. It should also show how many answers were correct & percentage (all in one page). 2) same folders with websites where I can mark areas and then ask people to click on the area that they think is most ugliest. This should also have a feedback option - when they click on the correct one then it highlights the area green (textbox: yes indeed - well done) if clicked on the wrong one then highlights red (textbox: reason why wrong).
