Pure PHP/css data visualisation page

Firstly this must be built in pure PHP (no framework), but using MVC principals (so using model file for queries, controller to handle the page and view to render the page). Any css and js must be in a separate file/folder. You must build on your own environment and provide a link for testing. You will not be given access to our environment. The basic requirement is to create a visually pleasing page that visualises the results of a dynamic questionnaire whose information is stored MySQL tables. The questions, question type and answers are all stored in the table and the visualisation must use logic to determine the question type and therefore the style of chart / output to show. There are 2 parameters that must be passed in url. ?film_id= &location_id= film_id is mandatory, but location_id is optional parameter Based on a well constructed query, you will display the summary results in graphical format for the given film_id If location_id is passed in URL then isolate to that location_id). Questionnaire title: from seeitfirstV2_questionnaire table Location ID: "All" (or if location paramater passed then display location ID value For each question, display the question number and question name (from seeitfirstV2_questionnaire_questions table) if answer_type=radio or select, then display results in pie chart format (colour options should be set in separate css file) if answer_type=slider results as a single numeric average if answer_type=checkbox results as a bar chart with percentage of responses including each option if answer_type=textarea or text, display as a scrollable text box to see all responses available I will provide the table structures and sample data for you.
