PHP/MySQL From Development (2 forms)

I need two PHP forms developed form one is simple, form two a little more complicated as it contains a check, possible subtraction from a field, and updates to about 4 other fields within the database.. BACKGROUND: I use coinhive that allows website users to mine for coins via my website, then based on the number of hashes they can turn the hashes into membership to my site. Form One: The user enters a username, the form then results to display the username and number of hashes. Both Username and Hashes are stored in MySQL database. Form Two: The user enters his/her username and selects one of four predefined memberships. The script then checks the username and number of hashes that user has. If the number of hashes the user has is greater than the number required (predefined) it credits that username with a 1,15,30 or 90 day membership based on the predefined hashes and subtracts that predefined number from that user's hashes. If the member does not have enough hashes the script will let the member know, "For the selected membership you do not have enough hashes, keep mining". If the user has enough hashes the membership is granted and subtracted from the total number of hashes that member has. 40 Million Hashes = 1 day free membership. (40,000,000) 270 Million Hashes = 7 days free membership. (270,000,000) 390 Million Hashes = 15 days free membership. (390,000,000) 590 Million Hashes = 30 days free membership. (590,000,000) 1.15 Billion Hashes = 90 days free membership. (1,150,000,000) Database is setup as follows: table: user -- field: username (checks) -- field: hashes (checks / subtracts if used for membership) table: ktvs_user -- field: username (checks) -- field: status (credit to premium if enough hashes) table: ktvs_user_subscriptions -- field: user_id (is associated with [login to view URL]) -- field: subscribed_object_id (updated if member has enough hashes) -- is based on 1,7,15,30 or 90 mbrs. -- field: subscribed_type_id (updated if member has enough hashes) -- is based on 1,7,15,30 or 90 mbrs. -- field: added_date (updated if member has enough hashes---date as of the moment the request is made). I will provide mysql info, subscried_object_id order, subscribed_type_id as each one is associated with the 1,7,15, 30 or 90 day memberships. Make sense? I would prefer if both forms are on one page, along with the javascript that the user uses to mine.
