Manage database on line with pass

I need to have and manage my database from web page on line. I have domain and server. I use Wordpress. With two types of users: 1 manager "Pet" and 2 "Vet" with less access. They must have a password to access. The information is confidencial After you do the database, I need to import my regist from my Excel spreadsheet. The database consists of 4 data sheets as shown in the figure with their corresponding "fields". *Pet User: you can access to enter all the forms. And create queries and forms. *Vet user: you can access to enter form: "Consulta veterinaria". *Vet User: You can only check the following fields 17 to 26, 15 and 39, after entering SEARCH "Socio" field. For example, user "Vet" enters the web page, puts password, and then selects: "enter data" or SEARCH. In SEARCH, enter "Socio" of existing regist and the query returns the registry of fields 17 to 26, 15 and 39, of the "Socio" entered in SEARCH. If you do not understand something, let me know.
