I need a web page with a contact form. The contact form should work as following: The user of the page enter his name, his email address, a number, and his message. Based on the number he entered, the message he typed is sent to the email address attached to that number in the php code. For example, if the user entered number 1 the the script sends an email to the address first@[login to view URL]; if he entered number 2, it sends an email to second@[login to view URL] etc I'll provide the list of the email addresses for the numbers, there are 8 of them (1-8). Thank you.
