I have website [login to view URL] and i need some customization in this, plere refer this website www.novaivifertility.com. In [login to view URL] one button is there on top right corner (BOOK Appointment) after clicking this one form will appear from top to down effect, I want the same thing in my website [login to view URL] for our "MAKE An Appointment" Button. 1) Creat same form & same effect 2) Should not able to select current date or any previous dates for appointment. 3) Phone Number field : only numbers allowed and mini and max digits are 10. (if country india & set to India as default country). 4) Success message after submitting form 5) SMS & Email for both (to visitor who filled form and to 2 or more Admins) we will discuss SMS & Email content. 6) I Will Provide SMS API 8) "MAKE An Appointment" Button should appear on mobile devices same as www.novaivifertility.com. 9) Changes should appear on all pages.
