Gravity Form Custom Layout

We need a custom gravity form built that allows users to select options and show a results page upon submission - similar to this example: [login to view URL]

I would like the boxes to be options that the user can select. After a user selects the option, it will turn to blue with white, no hover element needed.
Optimally, we would also like each row to be a "slide" of the questionnaire. Have the button read "next question" and go to the next row. Let me know if that makes sense.
Finally, we will also need to be able to add elements before and after this section, I can do that later but information like name, email, etc.
We will also have it redirect to a page when completed that I will need to design.
