Create a wordpress contact form connected to API

This wordpress plugin will: 1. GET contact fields and custom fields with CURL via the REST api 2. allow the WP admin user to select which fields are shown in the form and which fields are required pass the contact to the API with the create function - there are 12 fixed contact fields plus custom fields 3. Allow the WP admin to place the form anywhere with shortcode 4.
