Database Backend - Website Front End Data Display / Entry Webapp

See attached sketch and description of app of how i would like it to look and work. Description : Database backend, i have the database containing all the data to work with. Web Based Front End: Has to show data from database in the following way: See attached sketch. admins/supervisors can login to the system and search/see statistics immediately. They can search by name, address, and filter by polling station (one or all) (every user in the db is assigned a polling station which they belong to) When a search is made, the results come up below. Each result/user should have a vote option next to it, which you can assign Yes/Undecided/Other to that user and record it. (See sketch) This vote is a total that is to be stored, to be used for statistics on the left hand side. If a vote is already set for a user, it should come up with the appropriate check box checked in the results. Statistics should always come up on the left and updated as changes are made (See sketch) Administration: Admnistrator can login and see everything, and make changes to users in database or add a new record. Administrator can create supervisors, and assign them permissions on what they can edit (Supervisors need to be assigned to which polling stations they can edit users that belong to it, either one polling station or multiple. All supervisors can edit / change vote option ) Administrator assigns permission for the supervisor , the supervisor can edit users based on what polling division they are assigned to So for example you can say, supervisor1 can edit users from only polling division 2061, or multiple polling divisions or all, depending what he is assigned to. All changes made, whether by Administrator or Supervisor, needs to be logged in the database, this does not need to be visible in the main page and can be a separate page for the log, and is only viewable by admin. I have database of all the records already in MS SQL 2012 format, i also have it in mysql, let me know which you prefer. It is fairly straight forward, mostly pulling queries from the db and you need to add in the database tables for the VOTE option, and the log of changes.
