Custom Registration Form

Client wants us to design a web registration form that will allow students and clients to select courses, course locations, dates, and times. This form will need to be manageable in regards to adding and removing future courses. 1) Students (or clients) can input their identifying information - An email blast is sent to all of the clients -Agent clicks on link to see courses available with dates and time. (One course per day) -Agent provides required data -Name (Agent) -Address (Agent) -Company (Agent) -Contact number (Agent) -Email (Agent) -Purchase Order# (Agent) -List of Students 2) Choose the course they are interested in taking from a dropdown of our current courses -Agent chooses course for students 3) Choose the location they are interested in from a dropdown of our current locations -Agent will choose 1 of 4 cities available 4) View a calendar or list of times that we are offering that course at that location and allow them to choose one -Only one course available per day 5) Accept payment via credit card - Note: some of our agents have an account with us. We'd like the payment by credit card to be optional and to allow them to input an account number instead. - We currently have an online merchant set up to accept payments for our online courses so that part would be reusable. -Client would also like to add a feature for invoice paying agents. 6) Email response letting them know that they have successfully signed up for a course and with the course information in the email. We would want this to be extendable. For example, if we were to add new locations, we'd want to be able to add those to the location drop down. The same applies for courses. This website is built with ASP.Net and PHP. I am looking for this project to be completed by August 20th.
