Create a jquery mobile form where users can place multiple icons of various types on a map

Before an entreprenor can blast a part of a rock away, they need warn surrounding houses, put out watch posts to stop traffic, evacuate some buildings etc. All this needs to be planned, and this plan can be visualized on a map. See the attached example. The top right shows various icons that can be selected; once selected, a click on the map add that icon to the map. The bottom right shows specific information for a clicked icon on the map. For example when a neighboring house needs to be warned, the system should know the mobile phone numbers. The map should be able to show both earth and/or a map, ideally also with terrain. This form must be built inside an existing framework that downloads the form to an Android app. This app will load the form so the user can interact with the form. The app that loads the form offers some objects, such as gps location, database, and other objects.
