Build a Website

I want to help a company(ies) to register some of their employees to do some test/survey's. Here is an idea of what I am looking for: [login to view URL] I have done this sofar: [login to view URL] I want to use Jotform (Registered with them). They allow the data to be integrated with mysql and Google Sheets I need someone that can help do the following: a) Link the survey results of an employee to that of the company and/or Consultant? Can this prepopulated URL be used here? b) After completing a survey/test a results form must be e-mailed to consultant This test result form must however consist of a graphical analysis of the results, there will be some calculations on data i.t.o allocating a value to the answer and determining the % [login to view URL] Google Sheets help here? Alternatively, one must able to view it and then print it out. We are currently doing it on Excel - see attachment I have also included the link showing a typical test/survey and how a biographical form looks like. I want to avoid asking the whole biographical detail as per point (a). When an employee open this, it must be linked automatically to their company and consultant name. There must also be some kind of password access that after a person registered he/she can then have access to the forms.
