Add Clickbank upsell code to web page

I need a programmer to add code to a web page that asks a customer to verify they understand they are purchasing a subscription/recurring product. This is a requirement of the shopping cart I am using (Clickbank).
The following web page shows an example of the check box I need put on my web page. When the customer clicks on the payment link, they are not taken to the order form until they click on the check box. Scroll down to the bottom of this page to see the check box (the text next to the check box starts “I agree to the following payment terms”):

Note: my web page has been created using a Wordpress plugin called Thrive Content Builder. You will have to implement the code using this plugin.
