Online survey/profile generation and tracking system. Should be written in PHP tied to a MySQL database backend. Features must include - separate admin page to create new surveys, get individual record or compiled data, add questions, add weighted values to questions, establish question groups. Multiple question types must be supported - i.e., M/C, T/F, fill-in, cross-match. Ability to choose various scales i.e., best-to-worst , most likely-least likely, to represent question data. Must have ability to add survey participants and respondents by name and ability to assign login/access passwords individually including the ability to email login/password pairs to survey participants using template letter. Must have means to export database information to usable import format compatible with Microsoft Office products. Survey pages must be customizable through admin panel to add company logos graphics, etc. for different surveys. Generated HTML pages should be template based for easy modification by designers. Users must login, and choose which name or group they are filling out a questionaire on or they may view the data from survey results posted for themselves from respondents. Individuals results should be retrieved from database and dynamically be displayed by writing text tables and graphics using GD/Jpgraph PHP libraries - using Bar chart, spidergraphs, etc. If feasable, would like the ability to generate a downloadable PDF file for the individual from output html/graphics and have the capability to e-mail the file to an e-mail address of their choice. ## Deliverables
Complete and fully-functional working program(s) in executable/runable form as well as complete source code of all work done. Complete copyrights to all work purchased. complete working outline in common english langauge for Initial installation and user/administrators manual should be provided. All source code must be fully commented with common english language descriptions of ALL functions and variables. Code should be written to maximize modularity. Any source-code derived from other sources must be fully disclosed, programmer assumes all liability for using any code not in the "Public domain".
## Deadline information
A non-disclosure and non-competition agreement must be signed prior to proceeding with contract. A projected timeline for completion will be presented prior to project start for approval. Examples of comparable work will be presented for review prior to contract start. Payment schedules, if any, will be based solely on meeting projected project milestones. completion of project must not exceed 180 calendar days.
