I need a "simple" web-based app that will be written in PHP/MYSql.?
It should have 3 parts:
? ? ? ? ? ? ? ? user registration? -? ? Username,? Password,? email, Facility Affiliation,? Role
? ? ? ? ? ? ? ? Facilty worklist? ? ? ? -? ? worklist of all scheduled exams? abilty to add a new exam using a predefined form,? etc....
? ? ? ? ? ? ? ? Physcian login -? ? once an exam is created with that physcians name assigned,? the physcian can log-in and see all exams assigned to him and click a button that say's? "approve order".?
The basic idea is this.? ?
We currently use a x-ray order form for our mobile xray company.? facilities,? call us and we fill out the form.? ? we then dispatch our tech to go to that nursing home,? the tech perfomers the exam and sends it to the DR.? ? ? At some later point that order form is sent to the order physcian,? he must then sign it.? ? then we put it in the patients file.
We would like to streamline this process!? ? ? I would like an online way for facilities" nursing homes to be able to log in and see there schedule and order new exams.(each login can only see there facilty)? The new order be auto-emailed to one or more locations (pdf).? also once that order has been imputed into the app i would like the "ordering physcian" to be able to go in and "electronically sign" the orders,? and again after? the physcian approves the exam i would like a pdf to? be created and stored on the? server as well as an option to email it!(pdf).? Lastly we need a "admin" login that can see all schedule exams and manage dispatching.?
In-addition I need all of the? information from the orders to be stored in? MYsql,? both in its own db and? "some"? of the data from each record to? be? entered into another db on the same server.? all in? MYSQL.? ? ?
I will provide the form in it current form to make it easy to build the form!? ? ? The only tricking thing is that the first field on the form should be patientID and when a user puts in the patientID it should first look and see if that patient already exists,? if yes,? it should auto populate the patient info! ,? ? If no then the use can create a new patient!
