Form Processing with database and extras

Hello, I need a custom order form created that can be used ny multiple agents and each agent have access to their form contents via database log in. Example of form PHP script that could be used a starting point is here [login to view URL] My purpose is to set this up as a order form with special fields and post submission options. My team will be using this form to submit client orders directly. Each agent will submit sales using one form and have their submission logged unique to them. ON FORM ITSELF Full form creation functionaility so I can create multiple fields Allow users to add their email so form contents can be sent to them as well as a field where form contents are sent to client as receipt. On next page after submission, form summary is available with print option. File upload option - limited in size POST SUBMISSION Each agent has a database account (with normal database functionality) on server and can log in and view all orders, form content and open up and administer as follows: They can send form contents to any email --Not sure if this can be done post submission - There are text areas with labels I designate where agent can add comments (via sections I set up ahead of time), links, a company logo, header and then have option to create PDF and send this generated report to client via email. A unique page is generated on server for above report and agent can send email to client with link to this page Thank you, I look forward to working with you.
