Custom Web Application

We are in need of a customized web application using php, mysql and javascript primarily with a custom built database. The application will take the place of of a spreadsheet currently being used to calculate reserve studies. Attached are: Program 1st page file - showing the simplified page we will use to enter basic information on the company. [login to view URL] - this is the start of the database configuration Master component list - showing drop downs and informational fields to allow for selection of all components for each study being conducted. Full study - this shows the end result we are looking for based on the information input from program 1st page and master component list.
