110009 Contact form, with upload!

Hello, Im looking for a web/contact form. 1, as well as sending comments, it must be able to upload .zip files (upto 20mb). 2, Check that the form has been filled in correctly when submited. 3, easily be installed into a html page. For more full details and a couple of links to examples, please contact me. I dont mind if you already have a script ready or you have to code it from scratch i will be interested aslong as it works. I would also like to see any exaples you may have. Thanks
