So page 1 of the form will contain 2 dropdowns, with the exact same options as you can find on the link above. That data will be passed to page 2 or page 2b. If they choose fair credit or needs improvement, I want them to be redirected to a place (page 2b) where I can show them an affiliate ad for credit repair. If they click on the affiliate link, I want the affiliate link to open in a new window. I would also like them to be automatically taken to page 2. So I need a password protected page that allows me to upload an image and input the necessary data. LASTLY, I want all of the form data to be written to a database that I can access with a web interface and I also want the data to be sent to up to 5 emails. The data should be emailed to up to 4 lenders that are displayed on the final results page, plus an email-- with the data-- should be sent to me also. So that would be five. The form would pull up to 4 lender emails, and my email always. This will be an ongoing project. I will continue to tweak it and change it, so I would like a programmer that tends to be available. One last thing: I can purchase a PHP script maker to do everything I want to do here. I realize that it might be cheaper for me to buy the software. However, if it costs a LITTLE more, and I get personalized service, I am willing to farm this out. Ideally, I would find somebody who already has a script like this and just needs to build the data entry and data recovery web interface for me. Sorry this was so long, but I want to be very precise.
