Convert PDF to Mobi and Epub

I am looking for a company / somebody to write me some code based around php where i can upload a pdf file and convert it to Kindle format, Mobi and Epub. A basic set up with an upload filed on a form, the file is then uploaded and converted and saved in to a folder along with the original file, the locations of these files is then stored in a mysql database.
