Hello,

I am looking to get an editable PDF form designed using Adobe LifeCycle Designer. I have attached the layout of our new form in Microsoft Word format, as well as an example of exactly how I need the editable PDF form to look and work.

I will require the "Add Row" and "Remove Row" buttons within the form as well as the buttons at the bottom of the form (Save As, Reset Form, Flatten Form).

I will also require another 2 forms completed after this first task is completed and I am satisfied with the product.

I have been told that this needs to be created using Adobe LiveCycle Designer and NOT Adobe Acrobat DC, so please only contact us if you have experience with this software.
