We would like for this to be a fixed priced project by Phase based on agreement with the developer on expected time required. :::::::::::: Phase 1 Develop web interface (PHP) & database (mySQL) to collect and manage employee data with multiple employer log-ins Employers set-up employees via web interface and send links to employees via email to complete personal data survey Employees log-in to website (with link sent via email) and complete personal data (combination of multiple choice such as gender / religion and free text such as address - approx 35 data fields per employee) based on an excel list we provide of data required & formats. Employers can view & edit data for their employee. They cannot see other employers' employees / data (except site admin). :::::::::::: Phase 2: Create a tool to allow us to easily map data fields from Phase 1 to a set of forms via a HTML form generator or Adobe Acrobat auto-fill forms tool. :::::::::::: Phase 3: Integrate Phase 1 & Phase 2 to allow us to auto-fill forms for each employee. :::::::::::: Phases 4 - ...: To be briefed later.
