Dynamic Form using Swift 4 (programatically generate forms)

I want a developer to code a dynamic form UI for an ongoing project. This is a test job for a long term contract. You must have good experience with swift and ios programming in general. The form has to be created based on a JSON which will have a structure similar to the attachment. UI design will be provided to the selected guy. This form will have scrollable tabs and there will be two section of this form. We can discuss the details over chat.
A sample JSON for the form fields is included. There are multiple conditions to create this form elements based on the field_type parameter. PM for details.
