Style one single page of my web app -- repost

Hi, I have a web application designed in Perl/CGI which uses several CGI scripts. I need someone knowledgeable in Perl/CGI and good with CSS, JS and HTML5 to style one of my pages. I will show you the style I need and will will share the CSS too. I just need your help in modifying the HTML and CSS within my CGI script to give my page the exact same styling effect. If you do a good job and are good with PERL and CGI I will repeat hire you for styling the rest of the pages too. Looking for a prompt turn around time. Note: This project is for styling one single page only. Rest of the pages will be awarded to the right candidate as a separate project upon completion of this task.
