Professional Template Document for Small Business with Pre-Populated Sections that can be added or removed

I'm trying to minimize the time it takes my client to prepare an inspection report. I would like to provide a template that will have pre-populated sections that can be added or removed as necessary, and the ability to incorporate specific details (also pre-populated) related to those sections. The report can be anywhere from 3 to 5 pages and will include photos taken at the site to show damages or areas necessary for repair. I will provide the copy but I need someone who can deliver the finished product which should be user friendly. As this is not my area of expertise, I'm very open to suggestions on formatting and how the finished product should work, however please note that I want to avoid the use of any unique software that will need to be purchased by my client. Only bid if you have done this type of work before or something similar. I will need to see examples of the usable finished documents that I can then share with my client so he can make a decision as to what will work best for him. Also please provide your turnaround times.
