build a dynamic form

I need a dynamically created form: it needs to have 3 parts : 1) presents a data (long text) from a MongoDB database. 2) a dynamically created table, which rows and columns are generated from a MongoDB database (same as section 1, with the number of rows and colomuns changes from document to document). each crossing cell needs to be divided into smaller cells. names | name1 | name2 | name3 | etc name1 |-------------- | question1 | question1 | question1 | name1 |-------------- |question2 |question2 |question2 | name1 |-------------- |question3 |question3 |question3 | name1|-------------- |question4 |question4 |question4 | name2 |question1 | -------------- | question1 | question1 | name2 |question2 |-------------- |question2 |question2 | name2 |question3 |-------------- |question3 |question3 | name2|question4 |-------------- |question4 |question4 | name3 |question1 | question1 | -------------- | question1 | name3 |question2 |question2 |--------------|question2 | name3 |question3 |question3 |--------------|question3 | name3 |question4 |question4 |--------------|question4 | in the end, it needs to create a json dict. { names : name1, name2 ; question A : answer, question b: answer, queston c: date from database; question d :answer} that would save to a different mongodb colleciton if there is data regarding the question in the collection it needs to be auto-populated 3) another form : name1 (same as section2) question-a question-b question-c question-d saves to a different collection, if already exists, it auto-populates the fields.
