Login and a single data entry form into an existing database - Rush

This is a rush, please bid only if you can start right away and do it in a few days. You must be willing to work through the weekend. We just want what is specified to work, I will not add or ask for extra work. That will include finer greater details, only, in a document to be uploaded. Please bid only with the real price and state how many days you can have it completed in. Successful completion will be followed immediately by more work on the same thing. I will provide you with a database definition and a detailed specification document. This project will only be used by 100 pre-approved people, it is not for public use. This project is 2 functions: 1. A login page: username, password. Will NOT do forgot password function. You create test data: user/password only. (The login user will be created on another website, not this one. This is NOT part of this project.) 2. A data entry page for personal data: About 7 textboxes and 2 drop-down, each drop-down will be from a database table. The person who logged in will enter the data for a new customer record. One field will be a phone number. When the form is posted, you will send an SMS to the entered phone number and show an entry screen for the code. [SMS will not be done now, print code on screen for testing] When the code is entered, you will mark the new record as complete and enter the account number, with check digit, and a generated password into another table and then print these on the screen. There will only be one record for each phone number, if the phone number is duplicated and the sms code is entered, the phone number will be removed from the old account that had the phone number. We need the capacity to make this work in a second language, but this work will be done in a later project. The person who logs in (in part 1) and enters the data (in part 2) will be talking to the person whose data they are entering. The server side code will be in Python 3 with Flask and mySQL. Fewer dependencies is better. No graphics are required. Only plain, modern style forms. If you have any question before bidding please ask. I will be available all weekend on here
