Database & Front End w/ Search Form (searchable)

This is the link to the sample site I put up with an existing database and search form that is embedded from a paid 3rd party web app. [login to view URL] What the Client needs: o An editable database that he can manage (just some instructions on how to edit, input data and upload the new files basically) o The following 4 fields for the search form (like whats on the site); Name | Verification ID | Last 4 digits of SSN | Check Amount o The search query should pull up a section or page that basically \"verifies\" or confirms what the user is searching for after hitting a search button. o Then I just need a price and estimated time of completion. Example input data to be used in the search form that is on the site is attached, just so you can see an example of how the client needs it to function.
