I would like to create a web application based on the attached spreadsheet below. I think I would prefer a web application with an ecommerce site that can host the application as well as a video section. Each section would need its own login for multiple users. The videos and application would be different charges. There are some functions that I am not sure how to do within a spreadsheet that I would like to do with the web app.
1-- I would want it to be secure so I can give different people logins to access the app
2—I want to be able to choose teeth numbers like in the spreadsheet and have them auto populate on a printable page, like they do in the tabs on the spreadsheet. Each list of teeth numbers (or letters for baby teeth) corresponds to the different service (ei Fillings, crowns, implants, Bridges etc) Plus I want all the lists that are #2-15 and 18-31 to be changed to 1-32, so all teeth are represented. I previously left off the wisdom teeth (1, 16, 17, and 32)
3—The bridge and Implant tooth number lists do not currently populate on the printable sheet, but in the web app I would like them to be listed in the treatment recommendations section.
4—In the notes section I would like to just be able to freely type any extra notes in there.
5—the srp 1-3 and srp (deep cleaning) boxes should have 4 options each UR LR UL LL
6- What I currently cannot do quickly is the treatment sequence section. In the treatment sequence I would like once I have selected all the treatment recommendations to be able to quickly order each item into apt 1-8, and have them show up there. For instance if I chose to crown 30, and 3 and do a UR LR UL LL deep cleaning over 4 visits, I would chose those boxes. I could then chose what order each service is in 1-8. So I make #30 and UR LR as appointment 1. Anytime a crown or bridge is selected, it will need a seat appointment, that would be appointment 2. appointment 3 would be crown #3 and appointment 4 would be deep cleaning UL LL and seat crown. Those all would populate on the printable page in the treatment sequence and 4 appointments would be listed.
7—I would like to be able to set an interval between appointments maybe a drop down box to choose a number and another to chose days, weeks, month
8—The fourth tab is needed in the spreadsheet because I just don’t have enough room for adult and child services on one page. The tabs past that are to be translated to spanish (yet to be done)
9—I want to keep all of the existing functions of the spreadsheet just adding the above functions to it. The vital thing the spreadsheet does is provide the printable page for the patient. I would like all of the payment options present on the printable page, and I need to be able to edit the threshold dollar amount (currently $300) to show all the options. For example if a treatment plan is 299 or less currently in the spreadsheet it only gives the Pay as you go option. Over 300 it will give all four payment options. I like that but for individual users I need to be able to edit the dollar amount. I like the layout of the current printable pages, and I would like them to look very similar. The logo section maybe tricky, as If it is included we would need to be able to have each office be able to change to their own logo. If that is not easily possible it would be better to leave the logo off, and just be able to edit the name of the practice.
