Convert 10 Word documents with dropdown lists and selectable items to PDF

Have 10 word documents that have text boxes, drop-down lists and calendar selection fields. want these converted to fillable PDF maintaining the logo, drop-down lists, calendar selection etc.
an example is attached. please convert to PDF as part of bid application so can see if you are actually able to do it. none of the Word documents are more than 10 pages. will need a NDA agreed.
