The business provides case manager services (social services) to mental health clients, each team (Broward1) has 1 supervisor, each supervisor has 7 case managers and each case manager has X amount of clients, can be from 2 to 25 clients.
All clients have different insurances and each insurances granted us different amount of hours and each hour is convert in 4 units, the insurance pay us fee for services (per unit of 15 minutes), we pay our case managers the same way fee for services, some insurances they approved units per client the date of the admission, units last for 6 or 3 months (its important to have a section approval date and due date), another insurance we admit the client and we have to submit request after admission and we have to wait until insurance reply back to us with approval sometimes takes 2 days or 10 days, each time that I have a new client I need to input client's information, choose case manager assigned to case, choose team#, and input admission date, unit submission date, units approved, starting date and end date, each week we pulled from our software the units submitted per client, we will input that number for X week and automatically i want the excel the calculate the remaining units, on the same spread sheet we continue the same system weekly, that is the way how we verify if case manager is working with all their clients weekly (case managers must have to work with each client weekly). we also need a column in which we have to choose if client is active, or discharged, date of activation and discharged date as well, the report has also to calculate their total weekly productivity, a column with expected productivity by client and total expect productivity, if the system can convert next column in hours, next column a percentage expected is always 100% and units submitted in %. Also will be benefit to add colors, if the productivity is below 75% , or below 50% another color or the productivity was equal or above another color.
