Build a custom Google Form to be hosted on a website

I want to create a Google form that I can host on my website that asks a user a series of questions and then asks for the form to be filled out.

1. Club Name:
2. Email:
3. Product Number:
4. Total Quantity Required:
The answer to the quantity question will decide how many rows will then be displayed.
Column headings will be:
FRONT NAME, BACK NAME, FRONT NUMBER, BACK NUMBER, SIZE, QUANTITY
Size column will need to be sorted by smallest to largest based on sizes entered into the form. The size options will be selected from a drop down list of predetermined options.
The form will then need to total the Quantity at the very end end.
The form will then need a submit button that sends the contents to a preset email address.
