Interactive Excel Spreadsheet- Part 1 Programming, data validations, visio

I need a excel spreadsheet where people can etner things or select from dropdown where possible and based on the input, it should spit out what they need to note on the accounts or what process to follow: 1. Customer Name – Free text (Mandatory) 2. Address – Free text (Mandatory) 3. Customer Signature Confirmed - Yes/No dropdown with default to be ‘Select’ 4. Doctors Signature Confirmed - Yes/No dropdown with default to be ‘Select’ 5. Date Application received – Date Message if anything above is missing or not: Not eligible Dropdown now allows to select either life support or medical cooling in a dropdown [Form Type: Life Support/Medical Cooling] Once that is selected, it should have a dropdown which allows to select State: VIC/NSW/QLD/SA/WA Based on the input above: If Steps 1-5 Correct + VIC Concessions: Dropdown to select yes or no [Concession: Yes/No] If Steps 1-5 correct+ VIC + Medical Cooling + Concession Yes Output should be: Customer is eligible for concession Medical Cooling Operand added from: <whatever date application as received> Customer name: whatever was entered above Address: Whatever was entered above Customer signature: Yes Doctors Signature: Yes Letter Sent with confirmation of Concession Marked completed If Steps 1-5 correct+ VIC + Medical Cooling + Concession No Output should be: Customer is ineligible for concession Customer name: whatever was entered above Address: Whatever was entered above Customer signature: Yes Doctors Signature: Yes Letter Sent with confirmation of No Concession.
